Camp Policies & Procedures

Care of Students:
The studio is not responsible for providing before or after class care for students. Parents with students under the age of 5 are to remain in the studio during classes. Students are not to be left at the studio for excessive time before or after class. Students must wait inside the studio for pick-up.

 

Release of Liability

I, the ticket holder and/or being the legal and acting guardian representing a minor participant, hereby release, waive and discharge DA Dance Center, its directors, officers, employees, agents, independent contractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises, or in route to or from any of said premises, or any premises under the control and supervision of DA Dance Center. I understand that dance activities as conducted and taught at DA Dance Center have inherent risks of injury. I recognize that the participant is exposing himself/herself to such risks when undertaking dance activities. I, the undersigned and/or being the legal and acting guardian representing a minor participant assume and accept all risks of injury or damages resulting from such dance activities. I understand that appropriate physical contact is required during the instruction of dance, and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.

 

Medical Emergencies

I, the undersigned give permission to DA Dance Center, its directors, officers, employees, agents, independent contractors and volunteers to seek medical treatment for the participant in the event they are not able to reach a parent, guardian or emergency contact. I also agree that I will be responsible for any financial debt incurred by said action. I have declared on this form any physical/mental disabilities, limitations, restrictions, or condition and/or declare the participant to be in good physical and mental health.

 

Marketing Release
In conjunction with my participation I give permission for DA Dance Center to take and use photos and/or video of me or my child without remuneration in connection with studio publications, website, social media, and advertising. I understand that for the safety of our dancers and their families names will not be published or posted publicly.
